The Corporation Bureau has available a wide range of forms to assist individuals and business entities in filing information with the Bureau. The forms may be used to file new entities or to request changes in existing entities. These forms are now available for you to download (below) and are available in 'Adobe's Acrobat PDF' format.  The PDF format requires the use of 'Adobe's Acrobat Reader'.  You may also obtain the forms by ordering them through our voice mail system by calling (717) 787-1057, option 1. They are also available in person in the reception room or by telephoning our customer service representatives. Forms are also available through various stationery stores or from form supplier stores.

The Corporation Bureau offers a CD-Rom containing all corporate forms. The CD is prepared in Microsoft Word 2000 for Windows. Please mail the completed order form and $52 fee to the Corporation Bureau. The CD will be processed and mailed within five working days.
